These Terms of Service ("Terms") govern your use of the Logan Health website and GLP-1 weight loss consultation services. By using our services, you agree to these Terms.

Logan Health is a registered pharmacy (GPhC Registration Number: [Your Registration Number]) located at 209 Trafalgar Road, Greenwich, London SE10 9EQ.

2. Services Overview

We provide:

• Online health questionnaire for GLP-1 treatment eligibility assessment
• Pharmacist consultations (in-person or remote)
• Prescription of GLP-1 medications (Wegovy, Mounjaro) where appropriate
• Ongoing support and monitoring during treatment

3. Eligibility Requirements

To use our GLP-1 treatment services, you must:

• Be aged 18 to 85 years
• Meet the clinical criteria for GLP-1 treatment (BMI requirements)
• Not be pregnant, breastfeeding, or trying to conceive
• Not have contraindicated medical conditions
• Provide accurate and complete health information
• Be a UK resident

4. Health Questionnaire

The online health questionnaire is a preliminary screening tool. You must:

• Provide truthful and accurate information
• Disclose all relevant medical conditions and medications
• Understand that the questionnaire does not constitute medical advice
• Attend a consultation before receiving any prescription

We reserve the right to refuse service if we believe information provided is inaccurate or incomplete.

5. Consultations

All prescriptions for GLP-1 medications require a consultation with our pharmacist. During your consultation:

• Your health information will be reviewed
• Treatment options will be discussed
• Risks, benefits, and side effects will be explained
• You may be referred to your GP if appropriate

The pharmacist has sole discretion to determine whether treatment is appropriate for you.

6. Appointments

• Appointments must be booked in advance through our booking system
• Please arrive on time or join remote consultations promptly
• Cancellations should be made at least 24 hours in advance
• Repeated no-shows may result in service restrictions

7. Pricing and Payment

Current pricing (subject to change):

• Wegovy: From £149 per month
• Mounjaro: From £199 per month

Prices include:

• Medication supply (one month)
• Pharmacist consultations
• Ongoing support

Payment is required before medication is dispensed. We accept major credit and debit cards.

8. Medication Safety

GLP-1 medications are prescription-only medicines. You agree to:

• Use the medication only as prescribed
• Follow all instructions provided by the pharmacist
• Report any side effects or concerns promptly
• Attend follow-up appointments as recommended
• Not share your medication with others
• Store medication according to instructions

9. Refunds and Cancellations

• Consultation fees are non-refundable once the consultation has taken place
• Medication cannot be returned once dispensed (due to pharmaceutical regulations)
• If treatment is deemed unsuitable, you will not be charged for medication
• Refunds for unused services will be considered on a case-by-case basis

10. Limitations of Liability

While we strive to provide the highest quality care:

• We cannot guarantee specific weight loss results
• Individual results vary based on many factors
• We are not liable for side effects that are disclosed and accepted
• Our liability is limited to the cost of services provided

Nothing in these Terms excludes liability for negligence, fraud, or any liability that cannot be excluded by law.

11. Website Use

When using our website, you agree not to:

• Use the site for any unlawful purpose
• Attempt to gain unauthorised access to our systems
• Transmit malicious code or interfere with the site's operation
• Impersonate another person or provide false information
• Use automated systems to access the site without permission
